The MileOne Vision Plan

MileOne provides its employees with a vision plan through Davis Vision. This plan allows you to receive a variety of vision services, including:

  • Routine examinations
  • One set of traditional lenses and frames
  • Contact lenses once every 12 months

Participating vs. Non-Participating Providers

The MileOne Vision Plan may be used to receive care from either a participating or non-participating provider; however, using a non-participating provider will incur higher out-of-pocket expenses.

  • Examinations: There is no copay for in-network providers, and up to a $30 copay for out-of-network providers.
  • Lenses: In-network providers do not require a copay, but out-of-network providers charge up to $25-$60 for a copay, depending on the lens type.
  • Frames: For in-network providers, the expenses include no copay for fashion frames, a $20 or $40 copay for designer or premier frames and a $60 allowance for non-collection frames. Out-of-network providers charge up to a $30 copay for any frame type.
  • Contact Lenses: In-network providers have no copay for collection lenses and an $85 allowance for non-collection lenses. Out-of-network providers have a copay of up to $75 for collection lenses and, if medically necessary, a copay of up to $225 for non-collection lenses.

Extension of Care

  • The MileOne Vision Plan allows for coverage for both you and your dependents.
  • Employee Only: If you are the only recipient of care through the vision plan, the bi-weekly cost is a low rate of $2.16.
  • Employee + Dependents: If your vision plan is Employee + Children, Employee + Spouse, or Employee + Family, the bi-weekly cost is $5.60.
